Academic Suspension Appeal Form

To audit a course to not include in your grade point average or cumulative credit hours

To change your major. It is highly recommended that you speak with an Advising Specialist before changing your major.

Legal Permanent Resident/ Citizenship Documentation Affidavit

To complete your application for admission for the collection of Department of Homeland Security or other United States citizenship/nationality documents. Providing the appropriate citizenship/nationality documents may positively impact your tuition rates.

Consent for Release of Information

To give staff/faculty permission to release information to student’s parents or spouse

Consent for Release of Information - Cancellation

To remove those that you no longer consent to have access to your information

Graduation Application

To apply for graduation during the semester in which you plan to graduate. This link will redirect you to the Graduation tab for more information and a link to the form is provided.

Medical Withdrawal Form

To withdraw from classes if a student or an immediate family member becomes seriously ill or critically injured and the student is unable to continue attending.

Personal Information Update Form

Form to change your name, address, and/or telephone number.  Must present photo ID. Address and phone number updates can also be made under your Student tab/ Personal Information tab.
