Change of Major

To change your major. It is highly recommended that you speak with an Advising Specialist before changing your major.
Legal Permanent Resident/ Citizenship Documentation AffidavitTo complete your application for admission for the collection of Department of Homeland Security or other United States citizenship/nationality documents. Providing the appropriate citizenship/nationality documents may positively impact your tuition rates.
Consent for Release of InformationTo give staff/faculty permission to release information to student’s parents or spouse
Consent for Release of Information - CancellationTo remove those that you no longer consent to have access to your information
Graduation ApplicationTo apply for graduation during the semester in which you plan to graduate. This link will redirect you to the Graduation tab for more information and a link to the form is provided.
Medical Withdrawal FormTo withdraw from classes if a student or an immediate family member becomes seriously ill or critically injured and the student is unable to continue attending. This link will redirect you to the Vice President of Student Services tab for more information and a link to the form is provided.
