Academic Suspension Appeal Form
(Must be logged into MyJeffco for access.)
Each student is expected to make minimum academic progress while enrolled at Jefferson College. A student is considered to be making minimum progress if the student maintains a cumulative grade point average of at least 2.0.A student whose progress falls below minimum requirements shall be placed on academic probation. The student will be notified of the probationary status and informed of resources available for academic improvement.

If the student’s cumulative grade point average remains below 2.0 at the end of the probationary term, the student will be placed on academic suspension and will not be allowed to enroll or remain in classes for subsequent terms. The student must appeal the suspension and request academic reinstatement to enroll in subsequent terms.

U.S. Citizenship/ Eligible Noncitizen Documentation AffidavitStudents who indicate on their admission application that they are not U.S. citizens may be required to complete this affidavit. The form is used to provide official documentation verifying U.S. citizenship or eligible noncitizen status (such as a Permanent Residency Card or Certificate of Naturalization). Under Missouri’s out-of-state tuition policy, undocumented residents may be admitted if they meet regular admission criteria. However, in accordance with federal law and Jefferson College Board Procedure VII-001, undocumented students are not eligible to enroll in programs supported by federal funds. Until acceptable documentation is submitted, enrollment will be limited to general education courses.

Graduation ApplicationTo apply for graduation during the semester in which you plan to graduate. This link will redirect you to the Graduation tab for more information and a link to the form is provided.
Medical Withdrawal FormTo withdraw from classes if a student or an immediate family member becomes seriously ill or critically injured and the student is unable to continue attending.
Personal Information Update FormForm to change your name, address, and/or telephone number.  Must present photo ID. Address and phone number updates can also be made under your Student tab/ Personal Information tab.

Request for In-District Status FormTo petition for in-district tuition with proof of Jefferson County residency when out-of-district or out-of-state tuition initially.
Request for In-State StatusTo petition for in-state tuition with proof of Missouri residency when out-of-state initially.
Request to Prevent Disclosure of InformationTo prevent release of a student’s directory information.
Schedule Change FormTo add, drop or withdrawal from a course. Please reference the appropriate add/drop/withdrawal deadlines listed on the Class Schedule.
Tuition Refund Appeal FormTo appeal for a tuition refund and lab fees or removal of assessed tuition charges and lab fees during published refund period dates.
